For patients already established with Dr. Hoyle for the management of a mental health condition, please fill out the appropriate Current Patient form (under age 18 or ages 18-22) found in the dropdown menu at the Patient Registration heading.

For new patients, please fill out our New Patient Registration forms (under age 18 and ages 18-22), again found in the dropdown menu at the Patient Registration heading.

Our staff will be in touch to schedule an appointment. Our earliest expected opening date is mid-July 2026.

Note that Dr. Hoyle will not be providing general pediatrics care, so you/your child will need to maintain your relationship with your Primary Care Pediatrics Practice.
